The "Buy One, Get One" Medical Trend Physicians at the Bangladesh Shishu Hospital & Institute (BSHI) are currently observing an alarming and frustrating trend—a medical "Buy One, Get One" (B1G1) scenario. Parents are bringing their children into the clinic with primary complaints like a winter cough, fever, or stomach bug. However, upon physical examination, doctors are discovering the child has the primary illness plus a severe, highly contagious case of Scabies (স্কেবিস বা চুলকানি).

It is spreading wildly through schools, playgrounds, and homes.

Why Does the Itching Keep Coming Back?

Many parents complain that even after using prescribed creams, the child is waking up in the middle of the night scratching endlessly just a few weeks later. The reason? Treating the skin isn't enough—you must treat the clothes, the bedding, and the whole family simultaneously.

Based on the guideline prepared by BSHI, Dr. Rafia Rashid and her team at the Children's Gastroliver Center have developed a foolproof 14-Day Scabies Treatment Tracker to help parents completely eradicate these mites.

The Golden Rule of Scabies Treatment (Urgent Advice): If anyone else in the house has itching, the entire family MUST take the treatment at the same time, even if they don't have severe symptoms yet.

Phase 1: The First Attack (Days 1 to 4)

  • Day 1: Bathe the child thoroughly by rubbing the skin well. Before putting the child to sleep at night, apply the prescribed medicine all over the body, from the neck down to the toes.
  • Day 2: Give the child a normal bath during the day. Apply the medicine again at night from neck to toe before sleep.
  • Day 3: Normal bath during the day. Apply the medicine one last time at night from neck to toe before sleep.
  • Day 4 (The Final Step of Phase 1): Bathe the child thoroughly. Clothes: Boil all worn clothes on the stove for 10 minutes, wash them, dry them in strong sunlight, and iron them. Bedding: Dry all mattresses, pillows, and bedsheets in strong sunlight.

Phase 2: The Final Eradication (7 Days Later)

To kill any newly hatched mites, you must repeat the exact same process 7 days after completing Phase 1.

  • Day 1: Bathe well -> Apply medicine at night (neck to toe).
  • Day 2: Normal bath -> Apply medicine at night (neck to toe).
  • Day 3: Normal bath -> Apply medicine at night (neck to toe).
  • Day 4 (Final Cleanup): Thorough bath. Boil clothes for 10 minutes -> Sun-dry -> Iron. Sun-dry all bedding again.

When to See a Doctor

If your child's itching persists long after this 14-day routine, or if the scratch wounds become infected and ooze pus, it is time to consult a specialist.
